TERMS AND CONDITIONS: I acknowledge and agree to the following: I am authorizing payment today to Driven Consulting, Inc. for the program I have selected above, to my credit card that I have entered on this order form. This is a minimum 1 year commitment with auto-renewal after 12 months . Payments during the 1 year commitment period are non-refundable and there are NO cancellations or refunds, regardless of my use or participation, NO exceptions. Access to the program will be revoked 10 days after non-payment, and will be reinstated once payments resume in full. There are no make-ups should I miss or not show up for a class or service, and l will NOT be refunded or credited, NO exceptions. Auto-Renewal: I acknowledge and agree that following the 1 year commitment period , payments will be charged at the rate I selected above until I cancel. Cancellation requests MUST be sent in via email to Help@DrivenInc.com 72 hours prior to the next billing cycle, NO exceptions. Cancellation requests that are received after the billing cycle has ended will NOT be refunded, but will be terminated upon the ending of the next billing cycle. I hereby acknowledge the General Media Release, Earnings Disclaimer, Copyright and Confidentiality Agreement. Waiver of Liability: I acknowledge that a fitness program is a potentially hazardous activity. I should not participate unless I am medically able and have a safe in home area to move . I have consulted my physician and understand my risks and assume all responsibility for those risks. I agree to abide by any decision of Driven Consulting, Inc. relative to my ability to complete the program safely. I assume all risks associated with working out including, but not limited to, falls, pain, or death all such risks being known and appreciated by me. Having read this waiver and knowing these facts, and in consideration of my participation, I, and anyone entitled to act on my behalf, waive and release the Driven Consulting, Inc., their representatives and successors from all claims and/or liabilities of any kind arising out of my participation in this program. I grant permission to all of the foregoing to use any photographs, motion pictures, recordings or any other record of this event for any legitimate purpose. We reserve the right to deny the participation of any participant if, in the sole discretion of the company, such participation will be harmful to the participant, and/or disruptive to the company or other participants. No Refunds. I HAVE READ THE ABOVE RELEASE AND UNDERSTAND THAT I AM PARTICIPATING IN THIS PROGRAM AT MY OWN RISK.
